Medical Cannabis and Insomnia in Thailand

Difficulty falling asleep, staying asleep, or waking without feeling refreshed can affect concentration, mood, work, and quality of life. Insomnia may be short term, or it may become a persistent problem that needs a proper medical assessment.

Some people ask whether medical cannabis could help them sleep. The answer is not the same for everyone. Current research is still limited, and cannabis is not a first-line or guaranteed treatment for insomnia. A qualified healthcare professional should first review the cause of the sleep problem, current medicines, health history, and possible risks.

Understanding Insomnia

When Sleep Problems Need More Than a Quick Fix

Insomnia can involve:

• difficulty falling asleep;

• waking frequently during the night;

• waking earlier than intended;

• poor-quality sleep despite having enough time to rest; or

• daytime tiredness, poor concentration, irritability, or reduced performance.

Sleep problems can be related to stress, anxiety, depression, pain, medication, caffeine or alcohol use, shift work, menopause, sleep apnoea, or another medical condition. Identifying the cause matters because the most appropriate treatment depends on what is driving the problem.

For long-term insomnia, cognitive behavioural therapy for insomnia (CBT-I) is commonly recommended as the first treatment option. Healthy sleep habits and treatment of any underlying condition may also be important.

Evidence & Safety

What the Evidence Says

Can Medical Cannabis Help With Insomnia?

Research into cannabis, THC, CBD, and sleep is still developing. Some patients report changes in sleep or relaxation, but high-quality evidence is not yet strong enough to conclude that cannabinoid therapies are effective for sleep disorders in general.

Effects can also differ according to the product, THC-to-CBD ratio, amount used, timing, other medicines, and the individual patient. A product that causes drowsiness does not necessarily treat the underlying cause of insomnia or improve healthy sleep over time.

Medical cannabis should therefore be considered only after an individual assessment—not chosen from a general dosage recommendation online.

Safety and Suitability

Before any treatment decision, tell the practitioner about:

• all prescription medicines, supplements, and sleep aids you use;

• alcohol or other substances that may increase drowsiness;

• liver, heart, breathing, or neurological conditions;

• anxiety, depression, psychosis, bipolar disorder, or a history of substance dependence;

• pregnancy, plans for pregnancy, or breastfeeding; and

• work, driving, or safety-sensitive duties that require full alertness.

Cannabis and cannabinoid products may cause sleepiness, dizziness, impaired attention, mood changes, or interactions with medicines. CBD can also interact with medicines and may affect the liver. Do not drive, operate machinery, or mix products with alcohol or sedating medicines unless a qualified healthcare professional has confirmed that it is safe.

If you are pregnant, planning a pregnancy, or breastfeeding, speak with a doctor and avoid self-treatment with cannabis products.

What to Expect from THAIKANYA

Individual Review Before Any Recommendation

During a consultation, the care team may ask about your sleep pattern, how long the problem has continued, daytime symptoms, medical history, and current medicines. You may be advised to keep a sleep diary or seek assessment for another sleep or health condition.

If medical cannabis is not suitable, the practitioner may recommend a different next step or referral. If cannabis flower is considered clinically appropriate, an authorised practitioner may issue the required PT.33 prescription in accordance with current Thai rules. Approval is never automatic, and the product, quantity, and duration must be based on medical necessity.
